Registration documents:

1. Notarised copies of the child’s birth certificate /child’s passport.
2. Photocopy of child’s immunization and health records.
3. 02 passport-sized photographs of the child. (3cm x 4cm)
4. Photocopy of parents’ ID Cards /passport/ work permit.
5. Notarised copies of household registration book or temporary residential book.
6. 2 x Photographs (3cm x 4cm) of parents or care-giver authorised to pick up the child from the Centre.
